phantom-kv Enables Hot-Swappable Uncensoring via KV-Cache Injection

The phantom-kv system removes model refusals by injecting an 18MB learned bank of KV tensors into the cache instead of modifying weights. This allows for per-request, hot-swappable capability modes without altering the base model's checkpoints or quantization.
