A hybrid pipeline combining LLM-guided program evolution, exact evaluation, and Lean formal proof checking successfully discovered new entanglement-assisted quantum error-correcting codes. The method closed 54 gaps in archived EAQECC tables and produced 115 verified code records for prime powers q ≥ 3.
