A Multi-Agent Pipeline for Source-Grounded Synthetic Note Generation from Longitudinal Structured EHR

MedNotes uses a multi-agent closed-loop pipeline to transform longitudinal structured EHR into grounded clinical notes. The system employs generator, evaluator, and router agents to iteratively refine outputs, increasing acceptance rates from 69.4% to 91.4% on EHRSHOT encounters. Benchmarks show 0.980 mean factual accuracy and 0.028 critical hallucinations per encounter.
